ABOUT THE CENTER
The YHC Wellness Training Center of Your Health Concierge, Inc., offers advanced training for health and public health professionals to develop new knowledge and acquire or hone skills related to the practice of prevention and wellness. Our goal is to promote public health practice grounded in the latest research in health and mental health. Our trainers are at the forefront of research and practice in prevention. Our master trainers include public health professionals with backgrounds in behavior change, coaching, public health, ethics, nursing, psychology, medicine and social work.
We train leaders who want to improve their ability to use positive approaches to assist people to maintain a state of wellness in their lives. Our approach is motivational. Our trainings are designed to show health professionals how to use best practices from recent research on maintaining or restoring wellness in individuals, workplaces and communities.
Our training uses a small group, face-to-face approach to improving public health trainees practice and skills, allowing for time to discuss principles of wellness and to practice the skills we teach so that people who attend our trainings leave prepared to work with individuals, in workplaces and community organizations. We have a special emphasis on preventing and postponing chronic conditions in people as well as in helping them plan for how well they want to age.

Diabetes Prevention Program Lifestyle Coach Training
April 7-8, 2017 and June 9-10, 2017
Your Health Concierge, Inc., Wellness Training Center is excited to present the Diabetes Prevention Program Lifestyle Coach Training.
The two-day training is based on the curriculum developed by the Centers for Disease and Control and Prevention (CDC) National Diabetes Prevention Program, Prevent T2.
Core to the Prevent T2 curriculum is a Lifestyle Coach, the person responsible for leading sessions and supporting program participants during the 12 months of implementation. Lifestyle coaches are trained to use the CDC approved curriculum.
Attendance is required at both days of the training to receive a certificate of completion. The content will include:
- Overview of the National Diabetes Prevention Program
- Prevent T2 Curriculum
- Target Audience
- The Roles and Responsibilities of a Lifestyle Coach
- Group Facilitation
- Motivational Interviewing
- Recruitment and Retention
- Practice time
- Data Collection
Participants: Our training is available for anyone who is interested in bringing the Diabetes Prevention Program to their community or organization. We have a specific focus on training individuals who are linked to organizations that are applying for CDC recognition or are pending recognition by the CDC.
Format: The format of the course involves lectures, interactive discussions, and practice time.
Continuing Education Contact Hours: The Diabetes Prevention Program Lifestyle Coach Training is approved for a total of 13.5 CHES/MCHES Category 1 continuing education contact hours (CECH) (2 for advanced level) by the National Commission for Health Education Credentialing, Inc. (NCHEC).
